NHS Counter Fraud Services (CFS) will protect any information provided and will not share with any third parties, information which could identify you as the source of the allegation, unless required to by law.
At the foot of this form you can choose for CFS to keep in contact with you, completely, anonymously through a unique login - this enables CFS to ask any further questions about the information you have provided. If you have accessed this anonymous two-way communication system before, you can do so again using your existing login, via the link at the top right hand-side of this page. Check your junk mail box for any messages from CFS.
