Any personal information which you supply to CFS will be treated with the highest standards of security and confidentiality. CFS will not pass to third parties any personal information or additional information that could identify you as the source of the allegation, unless required by law.

At the bottom of this form you can choose to opt for NHS Scotland Counter Fraud Services to keep in contact with you - this enables us to ask any further questions about the information you have provided. The communication system is completely anonymous and operated through a unique login. If you have operated this anonymous two-way communication system before, you can access this again using your existing login, via the link at the top right hand-side of this page.

If your browser AutoFill or AutoComplete option is on it will complete some of the boxes with your standard information. Once you have filled in the form you have the option to print a copy before you submit it.

• If you have made an incorrect claim for exemption from NHS charges and have been issued with a reference number from the Patient Claims Team, you can make a payment online, using a debit/credit card. If you have any questions regards payments please email: nss.cfspct@nhs.net.
