As an employee of the fire service, this form is for you to use to give information on wrongdoing by those working within it, or you can call 0800 022 3818. Both services ensure you remain 100% anonymous; no one will ever know who you are, not even us. Should you wish to leave your contact details you can; these will then be kept confidential within your FRS.

Talking to one of our independent call agents or filling in this form will enable you to pass on what you know. Be sure to give as much detail as you are able to, as this helps build the full picture and ensures that the best use can be made of your information.
